1. This application has been formatted to allow you to type in the information. You may also, of course, fill in by hand.

2. The application allows for up to 2 children. If you have 3 or more children, please complete both our general application as well as our additional children application.

3. Once the information is completed and you have printed the application, you must sign and date the form and include a recent photo of your child(ren) and a copy of your child(ren)'s health card/insurance along with the deposit for each child.

4. If you wish to fax the application, you must send an original by mail/courier along with the photo and health card/insurance.

PLEASE NOTE
Last year's campers have first priority until Oct 16th. The date the application is received in our office with full deposit will determine the priority in terms of allocating remaining camper spots SO PLEASE GET IN YOUR APPLICATION AS SOON AS POSSIBLE.
